Information collected when you apply to join our network of health care professionals and medical facilities
surfsidestaffing.com provides information on how both Therapists and Medical Facilities can apply to take advantage of our services. During the application process, we collect a large amount of personally identifiable information that is required to process your application. This application information must remain on file at Surfside Staffing indefinitely so that we can successfully maintain our vital business records.
Surfside Staffing will never sell or distribute information collected during the application process to third parties for commercial purposes. However, for therapist applications, we must share information in the application with two parties: 1) prospective medical facilities, and 2) Liberty Alliance, Inc., the company that performs criminal backround checks on all our applicants. Application information must be shared with these two parties in order to successfully complete the application process and begin your employment.
You have the right to receive a copy of the report derived from your criminal background check. When applying, please indicate whether or not you would like to receive a copy of the report on the Surfside Staffing Therapist Application Form.
